Collabrios Health is a Lafayette, California healthcare technology company that builds a purpose-built operating system for home and community-based care, with a focus on PACE (Programs of All-Inclusive Care for the Elderly). Formed in October 2024 by private-equity firm Assured Healthcare Partners through the consolidation of RTZ Systems, PeerPlace Networks and Tabula Rasa's PACE EMR division, Collabrios combines electronic health records, care coordination, third-party administration and AI-driven analytics into a single integrated platform. It supports over one million individuals across 125+ PACE programs and roughly 1,000 home and community-based organizations nationwide.
Avenidas
Avenidas is a Palo Alto-based nonprofit that has helped older adults on the mid-Peninsula stay active, connected and independent since 1969. From its flagship enrichment center at 450 Bryant Street, plus the Rose Kleiner adult day health center in Mountain View, it runs fitness, arts and lifelong-learning classes, door-to-door transportation, caregiver support, the Avenidas Village aging-in-place membership, a Chinese Community Center and an LGBTQ Rainbow Collective - serving thousands of seniors and their families each year.
J. Thomas Briody
J. Thomas Briody is the President and CEO of Institute on Aging in San Francisco, a nonprofit serving older adults across the Bay Area and southern California. A third-generation elder care leader, he has spent more than three decades building integrated continuums of care across both for-profit and nonprofit sectors.
Michael Zawadski
Michael Zawadski is a healthcare technology executive who led RTZ Systems for over a decade before stepping up as founding CEO of Collabrios Health - a 2024 platform merger that unified electronic health records, financial TPA services, and care coordination tools for over 125 PACE (Program of All-Inclusive Care for the Elderly) programs nationwide. A JD-trained operator with an uncommon blend of legal, financial, and health-tech instincts, he oversaw the assembly of a company serving more than one million individuals in community-based care.
