The first problem after knee surgery is obvious: the knee hurts. The second is almost comic in its cruelty. At precisely the moment when moving is difficult, recovery begins to depend on going somewhere else to move. A patient must dress, negotiate the front steps, fold into a car and arrive on schedule for physical therapy. ROMTech starts with a quieter question: what if the useful part of rehabilitation came home instead?
Its answer is PortableConnect, a prescribed, motorized rehabilitation device that resembles a compact pedal station with a screen. It is only a machine in the same way a smartphone is only a telephone. Around the pedals sits a connected system: guided sessions, adaptive motion, a patient app, a Bluetooth angle sensor, video check-ins, a clinician dashboard, home delivery and human support.
The Brookfield, Connecticut company was founded in 2013 by Peter Arn and Sandy Gomberg. ROMTech now occupies a peculiar patch of healthcare real estate - part durable medical equipment company, part software platform, part home-service operation. That mixture is the point. A device alone cannot fix the gaps that open when a patient leaves the hospital. It has to arrive, fit, instruct, remember and report.
A machine that remembers the appointment
A physician first decides whether the patient is a candidate. A ROMTech field specialist then delivers PortableConnect, configures it and teaches the patient how to use it. Plans vary, but the public protocol commonly calls for three to five short sessions a day over several weeks. In the company-sponsored randomized knee-replacement trial, the experimental schedule was four or five sessions daily, six minutes at a time.
Six minutes is not an intimidating block of the calendar. That is a design decision as much as a clinical one. Traditional outpatient therapy concentrates effort into appointments, with travel and waiting wrapped around the exercise. PortableConnect distributes activity across the day. The machine can move the joint passively, assist the patient's effort, respond to active pedaling or add resistance as mobility returns. Its pedal radius adjusts to the range the patient can manage.
The patient does not have to maintain a spreadsheet. The app walks through the session and records progress. AccuAngle, a small Bluetooth wearable placed on the leg, measures knee flexion and extension. The clinician portal collects reports, adherence and health information. A care team can monitor the recovery curve, change a protocol and request a video check-in. The humble pedal becomes the front end of a remote-care system.

The actual product is adherence
Medical technology is often described through capability: what a sensor detects, how a motor behaves, which data reaches the cloud. Recovery is less obedient. People are sore. They are tired. Stairs look taller after surgery. A plan that works perfectly in a brochure can fail between the kitchen and the car keys.
ROMTech's practical advantage is that it treats those frictions as part of the product. The device is delivered rather than collected. It is fitted rather than merely shipped. Instructions appear during the session. Progress is visible. The prescribed work can happen without a commute. Accountability does not require the patient to become an amateur data scientist because the record accumulates in the background.

That model also explains who buys, uses and influences the service. The surgeon prescribes it. The patient pedals. A clinician watches the data. ROMTech handles inventory, warehousing, delivery and support; its surgeon FAQ says a provider's office is not charged for using the system. Insurers and public healthcare systems sit behind the economics. Pricing and the precise reimbursement mix are not publicly laid out, so ROMTech's business is best understood as a prescription-led care service wrapped around medical hardware.
Promising numbers, important asterisks
ROMTech says its pilot work found quicker return of range of motion and less reported pain than standard care. Its public materials highlight an average 112 degrees of range of motion after two weeks for PortableConnect patients, compared with 99 degrees under standard care. They also say half of device patients had recovered their range of motion by that point, versus roughly 13 percent of standard-care patients.
The distinction between a promising claim and settled evidence matters. The company sponsored a randomized, multicenter study comparing PortableConnect-only rehabilitation after total knee replacement with continuous passive motion plus home and outpatient physical therapy. The study enrolled 300 people and was completed in 2021. Its registry record was updated in July 2025, but results were not posted in that registry when this profile was prepared.
The 112-degree and early-recovery figures are presented by ROMTech from pilot studies. The larger 300-person trial is publicly registered as completed, but a registration record is not the same thing as a published result set. Patients should make decisions with their surgeon and insurer.
That does not erase the product logic; it sets the standard it must clear. In healthcare, convenience is useful, adherence is encouraging and a polished feedback loop is interesting. Comparative outcomes, safety, durable functional improvement and cost require evidence sturdy enough to travel beyond company marketing.
A category between the categories
PortableConnect is easiest to see by looking at what it is not. It is not a conventional continuous passive motion machine, which typically moves a joint without a full connected service surrounding it. It is not ordinary outpatient physical therapy, where a professional and patient share a room. It is not a software-only musculoskeletal program from companies such as Hinge Health or Sword Health. And it is more structured than a generic video visit.
ROMTech sits at the intersection: physical enough to move the joint, connected enough to send data, operational enough to reach the home, and clinical enough to remain prescription-controlled. That is harder to scale than an app. Every new geography needs machines, field staff, service, clinical relationships and payor pathways. It can also be harder to copy because the unglamorous work - routing equipment, fitting pedals, answering patient calls - becomes part of the moat.
The company reported treating 7,000 patients by November 2021, about a year after the first PortableConnect patient. By June 2025 it reported more than 121,000 patients across 44 states in sponsored Fast Company content. The same account said prescribing physicians in the second quarter of 2024 were up 93 percent from a year earlier. These are company-reported scale markers, but they describe a business moving beyond a tidy device launch into a national service network.
The federal door and the long game
One revealing expansion came through Lovell Government Services. In 2024 the companies announced that Lovell would serve as ROMTech's Service-Disabled Veteran-Owned Small Business vendor, putting PortableConnect on federal purchasing channels for Veterans Health Administration, Military Health System and Indian Health Service customers. It is an operational partnership disguised as a distribution announcement: Lovell knows the federal procurement maze; ROMTech supplies the rehabilitation system.
Veterans are an intuitive audience for at-home orthopedic recovery, particularly when distance, mobility or transport complicates follow-up care. The partnership gives ROMTech access to institutions that buy deliberately and demand process. It also widens the company's customer definition. The user may be one person pedaling beside a couch, but the purchasing chain can include a surgeon, hospital, federal contractor, agency and payor.
ROMTech has collected recognition along the way, including a place on Fast Company's 2025 list of innovative companies and, according to the company, the LexisNexis Top 100 Global Innovators list for 2026. Awards are punctuation, not proof. The consequential work remains less photogenic: getting a newly discharged person to complete today's fourth session and giving a clinician enough signal to know what should happen tomorrow.
The most ambitious thing about PortableConnect is not that it moves a knee. It is that the machine tries to keep the entire recovery conversation from going quiet.
Recovery as a daily operating system
ROMTech's mission is to connect clinicians and patients remotely through adaptive medical technology. In practice, that means turning recovery from a sparse calendar of visits into a dense trail of small actions. A patient sits down, pedals for a few minutes, answers prompts and gets on with the day. The care team sees another point on the curve. Tomorrow's protocol can reflect what happened today.
There is a product lesson here that travels beyond medicine. The best workflow may not demand more motivation; it may make the useful action smaller, closer and easier to repeat. ROMTech did not invent movement after surgery. It reorganized the movement around a patient's home, then connected the pieces that usually drift apart after discharge.
Whether that model becomes standard will depend on published evidence, coverage, clinician trust and the stubborn economics of putting hardware in thousands of homes. But its central observation is difficult to unsee. The living room is already part of recovery. ROMTech is trying to give it instruments, memory and a line back to the clinic.
