Amwise Diagnostic Built a Cancer Test for the Patients It Serves

Most breast cancer recurrence tests were trained on Western genomes. Amwise's RecurIndex was built on Asian ones - and its whole purpose is to help patients skip treatment they never needed. The founder learned the stakes the hard way.

In 2010, Sean-Lin Huang was 39 and running the kind of life that doesn't pause for bad news. Then came the diagnosis: stage III nasopharyngeal carcinoma. He went through chemotherapy and radiotherapy, felt what those treatments do to a body up close, and came out the other side with a question that would not leave him alone. When doctors decide how hard to hit a cancer, what are they actually going on?

For a lot of early-stage patients, the honest answer is: not enough. Guidelines, tumor size, a receptor status, and a professional hunch. The result is a system that tends to over-treat, because under-treating a cancer is the mistake nobody can live with. So people get chemo they may never have needed, and pay for it in money, hair, nausea, nerve damage, and months they don't get back. Huang founded Amwise Diagnostic in 2015 to shrink that guesswork - and to do it for patients who looked like him.

What Amwise makes

Amwise's product is called RecurIndex - 瑞可盈 in Chinese. It's a genomic test run on a sample of a breast tumor. It reads the activity of a panel of recurrence-related genes, folds in clinical factors like age, tumor size, and receptor status, and produces an estimate of how likely that specific cancer is to come back within five years. Armed with that, an oncologist and patient can have a sharper conversation about whether chemotherapy or radiotherapy is worth it.

The whole product in one line. Amwise's own logo is basically this equation - a small act of honesty in a field that loves a black box.

There's a detail here that separates RecurIndex from the pack. It doesn't hand back a single number. It reports two: RI-LR, the risk that the cancer returns near where it started, and RI-DR, the risk that it shows up somewhere far more dangerous. Those two risks call for different treatments, so collapsing them into one score throws away exactly the information a doctor needs.

Why "built for Asian patients" is the whole point

How it's different

The established names in this category - Oncotype DX, MammaPrint, Prosigna - are good tests. They are also mostly built and validated on Western populations. That matters more than it sounds. Research has suggested that Oncotype DX can overestimate recurrence risk in Asian patients, which is precisely the kind of error that pushes someone toward chemo they didn't need. RecurIndex was developed from the gene-expression profiles of Asian breast cancer patients to close that gap.

It's a simple idea with a hard moat behind it. "Works on average" and "works for my patient" are not the same claim, and the only way to earn the second one is to collect data most competitors never bothered to gather. That Asian genomic dataset is Amwise's real asset - harder to copy than any single algorithm.

The economics of not treating

Business model

Amwise sells RecurIndex to hospitals, oncologists, and pathology labs, typically as a lab service run on tumor tissue. It's a B2B diagnostics business: revenue per test, adoption driven by clinical evidence and physician trust. The pitch to a health system is counterintuitive but powerful - a test that sometimes tells a patient to buy less treatment can still save the system money, because a course of unnecessary chemotherapy costs far more than a diagnostic that rules it out. Amwise has published peer-reviewed cost-effectiveness analysis making exactly that case for hormone-receptor-positive, HER2-negative early-stage disease.

Small team, patient money

Funding & backers

The cap table is more impressive than the headcount. Amwise raised an early Series A with Alibaba Group, WI Harper Group, and Lippo Group, then closed a $10M Series B in February 2020 led by Jiangsu Simcere Medical Diagnosis - a strategic partner for expansion into Mainland China. For a core team that public records put at only a handful of people, pulling in that mix of investors is a signal: they weren't backing scale, they were backing a defensible dataset and a clear clinical need.

The company is registered in Singapore as Amwise Diagnostics Pte. Ltd., with much of its R&D and clinical work run out of Taipei and a growing presence in Mainland China. In 2021 it was named a Top 10 BioTech Startup in APAC. Its evidence base has kept building since - a validation study in Chinese male breast cancer patients, real-world cohorts of several hundred patients, and the prospective multicenter APRIL trial reading out recurrence prediction in early-stage luminal disease.

Where it goes next

Expansion

Breast cancer is the beachhead, not the ceiling. Amwise has said it is adapting the RecurIndex approach to colorectal cancer, with research interest in ovarian and pancreatic disease too. The playbook is the same each time: find a cancer where the treat-or-not decision is genuinely uncertain, and build the Asian-specific evidence to make it less so.

One case the company points to says more than any slide. RecurIndex was used to help guide treatment for a pregnant woman with breast cancer - a situation where every unnecessary intervention carries doubled weight. That's the win condition Amwise is optimizing for. Not a bigger number on a dashboard. A person who avoided harm they didn't need to suffer.

Precision medicine gets talked about as a frontier of more - more data, more targets, more intervention. Amwise is a reminder that the frontier also runs the other direction. Sometimes the most valuable thing a test can tell you is that you can stop. A cancer survivor built a company on that idea, for the patients the earlier tools weren't quite built for.
