Shreya Mehta spent ten years turning a treatment men dread into something they might actually choose

She reviewed devices at the FDA, then left to build one. The result is a small spring, a big idea about listening to patients, and a company that reached approval on its own timeline.

When Shreya Mehta was a kid, she took things apart. Not to break them - to see the logic inside, the way one piece pushed against another until something moved. She grew up feeling like an engineer, but the pull toward living systems never went away. That combination, the mechanical and the biological, is the through-line of everything she has built since.

Today she is President and CEO of Zenflow, a medical device company in South San Francisco that she co-founded in 2014. For a decade the company has worked on a single, stubborn problem: benign prostatic hyperplasia, an enlarged prostate that blocks urinary flow in tens of millions of men. The existing options ask patients to trade one discomfort for another. Mehta's bet was that they did not have to.

Chapter oneLearning the rules before breaking them

Mehta earned a master's in bioengineering from the University of Pittsburgh. While she was there she worked in the medical center's Artificial Heart Program, on the team responsible for patients living on mechanical cardiac support. It is hard to overstate how much that shapes a person. You are not sketching a device on a whiteboard. You are standing next to someone whose heart is being helped by a machine, and the machine has to be right.

The FDA recruited her out of that world. For roughly four years she served as a scientific reviewer, examining heart valves and artificial hearts - deciding whether other people's devices were safe enough to reach patients. Most founders talk about the FDA the way sailors talk about weather, as a force to survive. Mehta had sat on the other side of the desk.

That perspective would later become one of Zenflow's quiet advantages. She knew what reviewers actually needed to see, which questions they would ask, and why the answer to almost all of them was the same thing: evidence.

Chapter twoThe problem found her at Stanford

In 2013 Mehta was selected as a Thomas Fogarty Fellow at Stanford's Byers Center for Biodesign, a program built around a single discipline: start with the unmet clinical need, not the clever technology. There she met Nick Damiano, who would become her co-founder. Working through clinical problems, they kept circling back to BPH - a condition that is common, miserable, and served by treatments patients quietly avoided.

The team did something that sounds obvious and is rarely done well. They asked patients what they cared about. The answers were specific: no rigid scopes, faster recovery, and above all, no catheter after the procedure. Those preferences, not a lab breakthrough, pointed toward the design.

Out of that came Zenflow and the Spring System - a small, spring-like coil made from a shape-memory material that gently props the urethra open to restore flow. It goes in during an office procedure, needs no catheter afterward, and, unusually, it can be removed if the patient wants. Reversibility is a hard thing to engineer and an easy thing to underestimate. For a patient weighing a permanent decision, it changes the entire conversation.

Chapter threeA decade is a long time to hold a line

Zenflow was one of the first medical device startups Y Combinator ever backed, alongside early support from StartX and the UCSF Rosenman Institute at QB3. That is a strange fit on paper. Accelerators are built for software that ships in weeks. Zenflow was hardware, wrapped in regulation, on a path measured in years. The company leaned on rigorous clinical trials rather than speed, and Mehta has been blunt about why that mattered.

She started as chief technology officer, the natural role for the engineer who understood the device down to its alloy. As the company approached its regulatory and commercial phase, she stepped into the CEO seat. It was less a promotion than a matching of the moment to the person. The next milestone was approval, and few founders anywhere had spent four years inside the agency that grants it.

Chapter fourOne hand on the baby, one on the DocuSign

In November 2024 Zenflow closed a $24 million Series C, with new investor Cook Medical joining existing backers including Invus Opportunities, F-Prime Capital and Medical Technology Venture Partners. The money was earmarked for the company's premarket approval submission and the machinery of getting a product to market. The timing of the close is the part people remember.

She has told the sequence plainly: final terms came together Friday, last touches Saturday morning, induced Saturday night, daughter born Sunday, signatures circulating Monday. It reads like a stunt. It was not. It was what closing a hard round in the middle of a life looks like when both things refuse to wait. What she took from it was not a boast about stamina.

It is a rare thing for a CEO to name the scaffolding under the achievement instead of pretending it away. The story is not that she did it alone. It is that she was honest that no one does.

Chapter fiveWhy the small stuff was the whole point

Ask Mehta for a design principle and she returns to the same place. Not the coil, not the delivery system, not the trial data - all of which she can discuss in detail - but the patient sitting across from a urologist trying to decide. Every choice in the device traces back to what that person said they wanted.

There is a lesson in there for anyone building hardware in a regulated field. The instinct is to lead with the technology, because the technology is what took the years. Mehta's advice cuts the other way, twice. Start with the need. And when you do find the one piece of engineering that changes the game, do not bury it in modesty.

The kid who took things apart to understand them grew up to take apart a treatment category that patients had learned to endure, and put it back together around what they actually asked for. That is the quiet radicalism of Zenflow. Not a flashier device - a more patient one.
