CarelonRx and the last mile of medicine

A prescription is a promise. CarelonRx’s business is managing what happens next: the price, the pharmacy, the paperwork, and whether the medicine gets taken.

Ron had fourteen pills to take each day. Some came two or three at a time. In Elevance Health’s account of his care, he and his wife, Betsy, worried about missing a dose or mixing medicines up. The solution was wonderfully ordinary: a nearby independent pharmacy put the medicines into pill packs, synchronized the refills, and delivered them. The pharmacy was less than half a mile away.

The quick prescription
  • CarelonRx manages pharmacy benefits for health plans and employers.
  • Its strategy connects medical information, prescription data, and pharmacy support.
  • The test is practical: affordable medicines, reliable access, and fewer gaps in treatment.

ZipDrug, acquired by the business now called CarelonRx in 2020, had identified Ron through claims information and connected him with that pharmacy. The intervention addressed the tedious work between having a prescription and following it. Anyone who has tried to coordinate several refills will recognize the problem. A technically correct prescription can still leave its owner with a small logistics company to run.

The insurer builds a middleman

CarelonRx occupies that crowded space between the medicine, the insurer, the pharmacy, and the patient. It is a pharmacy benefit manager, or PBM. Employers and health plans hire it to organize prescription coverage: which drugs appear on a formulary, which pharmacies participate, how claims are handled, and when a prescription needs clinical review.

Its beginnings were corporate. Anthem announced IngenioRx in October 2017; operations began in May 2019. The business became CarelonRx in 2023, inside the company now called Elevance Health. Deepti Jain, its founding president, led its creation and launch. This was an insurer building a pharmacy business with an existing population to serve.

That parentage shapes its pitch. Connecting medical and pharmacy benefits can reveal problems that a prescription claim alone cannot explain. CarelonRx also serves external customers. Elevance has described pharmacy coverage partnerships with Blue Cross of Idaho and AlohaCare, extending the proposition beyond its affiliated health plans.

The competitive arrangement has a twist. CVS Caremark is an alternative PBM, alongside Express Scripts and Optum Rx. Yet CVS also supplies certain core pharmacy services to CarelonRx. Elevance’s June 2026 filing says that agreement extends through December 2027. A benefit buyer is choosing a combination of clinical strategy, contracts, and infrastructure; the names on those components can overlap.

The price of the whole picture

Consider another case Anthem published in its 2019 annual report. A member with seizures had stopped taking an expensive branded medicine. Clinical pharmacist Joshua Yelverton reviewed medical and prescription records and identified a generic option. Anthem reported that the member’s cost became $10 for a three-month supply. The important detail was the connection between affordability and the ability to continue treatment.

CarelonRx’s services give that idea an operating vocabulary. Rx Care Nexus looks for gaps in adherence, safety, and appropriate use, including opportunities for lower-cost alternatives. Specialty Condition Management adds nurse support for people managing complex therapies. BioPlus, acquired in 2023, brought specialty-pharmacy capabilities for conditions including cancer and multiple sclerosis.

The less charming tools matter too. Prior authorization requires a clinical review. Step therapy can require trying an alternative medicine first. Quantity limits control how much is dispensed. These mechanisms help sponsors manage use and expense, while introducing requirements that members and prescribers must navigate. The experience depends on how those requirements are implemented.

The business is substantial. Elevance reported $43.4 billion in CarelonRx operating revenue for 2025, up from roughly $36.0 billion in 2024. Those are segment revenues, including affiliated business. They measure business activity, not member savings. Sponsor contracts and benefit designs determine the economics of an individual plan; there is no single retail price for the service.

The clinical evidence needs equally careful reading. Elevance’s 2023 reporting described 30% fewer inpatient admissions among Medicaid members supported through a CarelonRx and CPESN community-pharmacy partnership over a two-year evaluation. That is a company-reported finding for a particular program and population. It gives buyers a question to investigate, rather than a percentage to paste into every forecast.

January 2024: the handoff slips

The operational promise met a public test when CarelonRx Pharmacy launched its digital home-delivery service on January 1, 2024. A January 22 FAQ acknowledged system issues and high call volume, with longer waits and delayed responses to member questions. Convenience had acquired a queue.

For one sponsor, the problems changed the rules. In March 2024, ACWA JPIA suspended its requirement that Anthem PPO participants obtain maintenance medicines through mail order or designated alternatives, allowing in-network retail fills through year-end. It cited ongoing home-delivery issues. This was a concrete response to an access problem: preserve another route to the medicine.

The lesson travels well. Any service that centralizes a necessary task needs a workable fallback when the central channel stalls. In pharmacy, delivery and customer support are part of access. A useful application cannot compensate for an order that fails to arrive.

What happens after the click

For members, the everyday tools are straightforward. The CarelonRx app offers drug pricing and coverage checks, pharmacy search, refill requests, order tracking, and service chat. Availability follows the member’s plan. A national network directory is a starting point; the pharmacies and medicines covered by a particular benefit still need checking.

The same thinking appears in its weight-management program. An update scheduled for October 1, 2026 adds Noom-supported digital engagement alongside lifestyle and medication support where applicable. CarelonRx specifies that the digital experience does not prescribe or dispense medication. Coaches, reminders, and navigation have defined jobs within a larger service.

The useful idea to copy is precise: find the obstacle between an intention and a repeated action, then assign someone to remove it. Data can identify a missed refill. A pharmacist can discover why it was missed. A local delivery, a simpler pack, or a different covered option can make the next dose possible. Ron’s nearby pharmacy makes the point without a dashboard.
