There is a moment repeated a few billion times a day that medicine mostly ignores. You use the bathroom, you flush, you move on. Inside that flush is a running readout of how your kidneys, bladder and metabolism are doing - and it disappears down the drain. Starling Medical looked at that vanishing data and decided to keep it.
The company, founded in Houston in 2019 and part of Y Combinator's Winter 2022 batch, makes a reusable device that clips onto a standard toilet and analyzes urine at each visit. Most remote patient monitoring depends on a person doing something. Strap on a watch. Prick a finger. Log a number in an app before bed. The trouble with all of it is human: people forget, get bored, or quietly stop. The monitoring is only as good as the habit, and habits are fragile.
Starling's design sidesteps the habit entirely. You already use the bathroom. The device sits there and does the work while you do nothing new. Recurrent UTIs are a useful example of the stakes. They are common, they recur precisely because the underlying pattern goes unmonitored between flare-ups, and they drive a steady stream of clinic visits, antibiotics and, in the worst cases, hospital stays. Starling's approach treats the gaps between appointments as the part of the story worth watching. The company also frames the same tooling around antibiotic stewardship - if you can see an infection turning early, you can treat it more precisely and prescribe less blindly.
For the patient, the day-to-day is meant to be almost invisible. Download the app, clean the toilet, snap the anchor in place, and then carry on. The insights surface on a phone as trends rather than raw numbers, with nudges toward the small behavioral changes - hydration, timing, follow-through on a prescription - that keep a manageable condition from becoming an emergency. The point is not to turn every user into an amateur lab technician. It is to make the monitoring disappear into the background until the moment it matters.

Prevention has a chronic funding problem: nobody gets paid much to keep a patient from getting sick. Starling's answer is to route its revenue through remote patient monitoring reimbursements. Physician groups and insurers are the partners; the monitoring service, delivered through those providers, is billable. That flips the usual math, so keeping patients healthy becomes a source of revenue for the practice rather than a cost center.
The model got its first real test with a large private urology practice in Texas - one with roughly 200,000 patients - that signed on as an early customer. For a company selling a recurring monitoring service, a single partner at that scale is less a sale than a proof of concept for the whole business.
It is a B2B2C structure with direct-to-consumer texture. The provider is the channel and the payer relationship; the patient is the person who actually installs a device in their bathroom and opens the app. Starling has also described its offering as the first FDA-registered at-home urine diagnostic service - a regulatory footing that matters a great deal when your customers are physicians.
Starling was founded by Alex Arevalos, Hannah McKenney-Klepcha and Drew Hendricks. McKenney-Klepcha came from product engineering roles at Medtronic and Johnson & Johnson - the kind of pedigree that teaches you exactly how hard it is to ship a regulated medical device, which may be why the team chose a problem most software founders would never touch. Geography helps. Starling's office sits on Holcombe Boulevard, inside the orbit of the Texas Medical Center, the largest medical complex in the world. When your product needs clinicians, urology practices and regulatory savvy, being down the street from all three is not a small advantage.

The 2023 seed round - $3.4M led by Rebel Fund - drew a long list of backers including Y Combinator, Innospark Ventures, AI Basis, Capital Factory, Coho Deeptech, Magic Fund, Gaingels and several family offices. Public trackers put the company's total raised in the range of $11-13M, with an additional reported tranche in early 2026 as it kept scaling the monitoring service.
Starling sits in an odd, defensible gap. Lab-based urinalysis is accurate but episodic - you only get a reading when you show up. Wearables are continuous but can't see what urine reveals. A handful of connected-urinalysis efforts have appeared around the same idea, but Starling's wager is that the winning version pairs the sensor with a clinical team and a reimbursement path, not just a consumer dashboard.
The competitive set reflects that bet. Consumer-facing connected urinalysis products have arrived from larger hardware companies, and lab and kidney-test services are digitizing the same category from the clinical side. Starling's differentiation is not a single sensor spec; it is the decision to own the whole loop, from the fixture in the bathroom to the nurse on the phone to the reimbursement code that pays for it. Each piece on its own is copyable. The integration is the harder thing to replicate.
There is also a defensibility argument in the boring details. Shipping a regulated device that survives a bathroom, talks reliably to a phone, and clears FDA registration is slow, expensive work that scares off software-first competitors. That difficulty is a moat as much as a burden - the same reason the founders' medtech background reads less like a resume line and more like a prerequisite.
Whether at-home urine monitoring becomes as ordinary as a bathroom scale is still an open question. What is clear is that Starling has picked an unglamorous, overlooked corner of diagnostics and built the full stack around it - hardware, software, clinicians and billing. In a field crowded with apps that hand you data and wish you luck, being the one that calls you back is a real position to hold.
